Guatemala 2010 - How to apply

To apply for the 2010 Guatemala mission

If you are not an existing member please click here to fill out a membership application. You will then receive email instructions on how to proceed.

If you already have an H4H account user name and password please click here . You will need to enter your user name and password to move to the on line registration form if you are not already logged in. Once we receive your on line application you will receive email notifification of provisional acceptance to the team pending submission of your payment and documents.

2 ways to pay your Membership And Mission Deposit

By Credit Card

Once your application has been accepted you can pay your $25.00 H4H Annual Membership Fee and $500.00 mission deposit by credit card using the donor link OR

By Cheque

Please mail a Cheque for $525.00 (payable to 'Health for Humanity Service Society') to H4H at 15745, 102B Ave, Surrey, BC, V4N 2M1, Canada. This becomes non-refundable once travel expenses have been incurred by H4h.

If you need to revise your application form at any time you can go to 'My Account' when you are logged in and then click on 'Mission Applications'.

Required Documents for medical professionals

New Applicants

  • Resume (max 2 pages as a word document)
  • Copies of your original certification (RN, MD, FRCS/P Specialist Diplomas) 

 All applicants

  • Copy of your 2010 provincial registration
