 Please register on-line now for our next surgical mission to the Philippines.  This is a 2 stage process. First you need to become a member of H4H ($25.00 annual fee). Then you need to fill out the application form.

If you are not an existing member please click here to fill out a membership application. You will then receive email instructions on how to proceed.

If you already have an H4H account user name and password please click here . You will need to enter your user name and password to move to the on-line registration form if you are not already logged in. Once we receive your on-line application you will receive email notifification of provisional acceptance to the team pending submission of your payment and documents.

2 ways to pay your Membership

By Credit Card

Once your application has been accepted you can pay your $25.00 H4H Annual Membership Fee and $500.00 mission deposit by credit card using the donor link OR

By Cheque

Please mail a Cheque for $525.00 (payable to 'Health for Humanity Service Society') to H4H at 15745, 102B Ave, Surrey, BC, V4N 2M1, Canada. This becomes non-refundable once travel expenses have been incurred by H4h.

• fax or Email a scan  of your original certification (RN or MD, FRCS/P Specialist Diplomas) 

 All applicants

• Copy of your 2010 provincial registration
• Copy of your 2010 CMPA membership (physicians)
