If you are not an existing member please click on "Create New Account" on the right-hand side (in the log in box) to fill out a membership application. Please also pay your $25.00 membership fee by using the donate link.
After you have logged in, click here to fill out a mission application form. Once we receive your application you will receive email notification of provisional acceptance to the team pending interview with team leader and submission of your payment and documents.
2 ways to pay your Membership And/Or Mission Deposit
By Credit Card
Please pay your annual membership fee of $25.00 immediately through our Donate and Paypal links for consideration of your application.
Once your application has been provisionally accepted you will receive emial notification. Please submit your $1000.00 mission deposit by credit card using the donor link OR
By Cheque
Please mail a Cheque for $1000.00 (payable to 'Health for Humanity Service Society') to H4H at 15745, 102B Ave, Surrey, BC, V4N 2M1, Canada. This becomes non-refundable once travel expenses have been incurred by H4H (early September).
Required Documents for medical professionals
New Applicants
- Resume (max 2 pages as a word document (emailed to info@h4h.ca)
- Copies of your original certification (RN, MD degree and LMCC, FRCS/P Specialist Diplomas) scanned and emailed to info@h4h.ca
All applicants
- Copy of your 2012 provincial registration ( nurses we will check your provincial registration on line) scanned and emailed to info@h4h.ca
You will recieve email confirmation of successful application once your interview is completed and deposit payment and documents are received.
